The book was inspired by the 1928 Motherhood in Bondage, a collection of letters written by women to Margaret Sanger, the founder of Planned Parenthood. Publishing these letters from women in desperate circumstances helped to equate the concept of birth control with higher values of wanted children, healthy mothers, loving couples, self-realization, and female emancipation.

She goes on to present a case study of Rhode Island, one of only two states to exempt women from prosecution for abortion. When most states adopted Comstock laws and eugenic sterilization legislation, partly to control and shape the population, Rhode Island did not. Caron also examines the political, moral, and economic forces that shaped reproductive policies and the impact these restrictive policies have had on women's autonomy and equality.

Yet women in many countries suffer from laws, practices, customs, and cultural and religious norms that consign them to a deeply inferior status. Ross examines international and regional human rights treaties, including treaty language and the jurisprudence and general interpretive guidelines developed by human rights bodies.
