Description

Having acknowledged that prostitutes have professional and personal difficulties in using conventional services, a medical outreach project was established in November 1990, tailored specifically to their needs. The aims are to reduce HIV transmission and the incidence of STD. A team of a doctor and a health visitor (H V) work at a drop-in centre for female prostitutes one evening and one afternoon session per week. Full STD and HIV screening and treatments are available. The HV offers individual counselling which can encompass a wide range of issues, such as HIV, termination of pregnancy, drug misuse, and support through personal crises. The HV encourages the uptake of free condoms, and gives advice on their use and safer sex practice. The HV can offer antenatal and postnatal assessments. Eighty three women have attended the clinic for medical examination, and many more for advice. Topics most often raised with the HV include contraception, hepatitis vaccination, child care advice and well woman issues. The average number of condoms taken by a woman at each session was 45. Prostitutes want to take responsibility for their own health when the service offered is accessible, flexible, and non-judgmental. Shared knowledge and information among the women about, for example, hepatitis B vaccination, suggests increased self-awareness and self-esteem.
